Juvenile Justice Case Manager - Contractor






The 19th Judicial Circuit is seeking to contract with a qualified candidate for a 30 hour per week Juvenile Justice Case Manager at an hourly rate of $16. The Juvenile Justice Case Manager will provide services to families in the Civil Citation, Juvenile Domestic Violence, and Comprehensive Assessment and Intake Programs in the 19th Circuit. The Case Manager will be expected perform comprehensive intake assessments, development of a case plan, identification of services to assist families in addressing the needs identified in the case plan and monitor ongoing compliance throughout the length of the contract. Examples of these functions are:

Perform intakes

• Assess youth’s and family’s needs

  • Explain program requirements and option for youth/family to refuse service
  • Elicit youth’s and family member’s perception of situation and needs
  • Complete parent and youth questionnaire, and youth substance abuse questionnaire
  • Have youth complete Beck depression scale and/or MASII
  • Initiate drug test, if appropriate
  • Ask relevant follow-up questions
  • Complete need assessment document clearly indicating youth’s/family’s needs and explain any unusual extenuating circumstances
• Develop a treatment plan that identifies services to meet assessed needs.

  • Clearly identify all services/projects required on treatment plan/contract
  • Include timeframes when necessary
  • Review insurance issues
• Link family to specific service providers

  • Provide specific contact information for accessing services
  • Provide clear information regarding nature of services provided by agency
  • Make referral for family if needed, especially if situation calls for immediate intervention

Ensure youth/family receives needed services

• Help family access funding for services such as a Medicaid application
• Advocate for family with service providers when necessary to ensure client needs are addressed such as:
  • Ensure referral to outside agency is acted upon
  • Obtain expedited intake when needed
  • Ensure quality of service, such as frequency of contact and variety of interventions (e.g. counseling, psychiatric evaluation, medication management,) to meet client’s needs
• Monitor family compliance with treatment plan and quality of services provided by agency
  • Document compliance through contact with service provider not just youth/family
  • Determine not only client's participation but commitment to service such as regular attendance and progress in treatment.
  • Determine youth’s/family’s satisfaction with services being provided

Document all case activities in a professional manner

• Maintain neat and orderly case records.
• Maintain up to date case notes regarding all pertinent information relevant to the case.
• Enter and update all information into the Juvenile Justice Database.
• Ensure that all forms are completed neatly and in a timely manner
• Conduct JJIS check prior to final hearing to ensure no new LEO contact/arrests.
• Close out case in JJ database.
• Send case closure notification to State Attorney’s Office on each matter in a timely manner

Minimum qualifications for the positions are a B.A. degree in a social/behavioral science and one year experience working with families in a social service agency or an A.A. degree and two years of experience.
